Service Agreements & Intake Forms
It is recommended by the National Disability Insurance Agency (NDIA) that all NDIS participants have a written service agreement with their providers. As such, Gawler Psychology will be introducing a service agreement for our NDIS participants. Our NDIS service agreement outlines what supports will be delivered, how they will be delivered, our terms or service, the cost of supports, our dispute resolution process, and the responsibilities of the provider and client.
